Sickness Certificate

CORONAVIRUS - If you need to self-isolate for 7 days, you do not need a certificate from your GP.  Please use the link below for a self-certificate for the first 7 days of your illness. If you have been told to stay at home and need an isolation note please see the second link below

You may request a repeat sick certificate (non Covid-19) via our generic email greccg.coldharbourhillpms@nhs.net

Please note you will need to see a GP for your first certificate for sickness over 7 consecutive days.

Please include the following details:

Name, Date of Birth, Contact telephone number, Date for certificate to begin, Date for certificate to finish/how long for and Reason for sickness.

All certificates will be available to collect at our main surgery 79 William Barefoot Drive SE9 3JD.Between 12.00-14.00.

Please allow 2 working days for certificate to be ready.
